I think this is a money clip, a bit fat for a book mark, could be a tie clip.

The blade is inlaid with a black stone
very ornate hilt, I used to think it was broken on one side but I don't think it is, very smooth break and looks fine as it is, your choice.
marked kelantan on the back
it looks like sterling silver to me but I cannot find a mark I recognise.
